What are the eligibility criteria for the Structured Family Caregiver Program?
To be considered for the program, your family must meet the following conditions:
- The client (the disabled) must meet Georgia Medicaid financial and medical criteria, as set forth, under the Community Care Services Program (CCSP) or SOURCE Program.
- The client must live with the family caregiver.
- The caregiver cannot be a person legally responsible for the client.
- The client must require at least 5 hours of activities of daily living assistance and care.
To be qualified as a structured family caregiver, you need to be:
- At least 18 years of age
- Has a clear criminal National Background Report
- Willing to care for a disabled or sick patient with a positive mindset
- Must not be employed during the hours of care for the client, including self-employment.
